Join to help ensure the Connect2 Community Information Exchange is informed by and responsive to the community through the development of network standards, partner engagement strategies, partner onboarding, and systemic reviews of network data to identify resource gaps and opportunities to improve care coordination. It is also a space for organizations to connect and share resources, information, and lessons learned.
Meets bi-monthly on the 2nd Wednesday of the month. 10 –11:15am
