Connect2 was co-designed with the communities and frontline partners who use it, and that process has never stopped. The Connect2 Impact Alliance is the partnership that carries that work forward: three organizations with complementary strengths, one shared commitment to whole-person care.
HealthierHere co-designed Connect2 and leads its governance and long-term stewardship. Ready Computing provides Channels360, the interoperable technology behind secure, consent-based information sharing. Unite Us operates the closed-loop referral infrastructure connecting community-based organizations, health providers, and public agencies.
Together, the Alliance offers a Community Information Exchange that is community-centered, nationally proven, and operating today. Connect2 is refined through years of daily use by the organizations that depend on it, and designed to plug into the tools partners already run. Whatever an organization arrives with, it can take part.
The community-rooted lead. HealthierHere is the Accountable Community of Health for King County and a neutral nonprofit convener across community-based organizations, Tribes, health providers, managed care organizations and government agencies. Over nine years of community co-design, including two years devoted to the consent model alone, HealthierHere built Connect2 with the communities it serves and continues to lead its governance.
The interoperable foundation. Ready Computing's Channels360 platform powers Connect2 Exchange, enabling secure, standards-based information sharing across participating organizations. Ready Computing brings Community Information Exchange experience from New York's Health Equity Reform Waiver and Georgia's statewide CIE infrastructure, along with high-volume health data exchange across federal and state environments.
The accountable community connection. Unite Us extends Connect2's reach through a statewide closed-loop referral network already used by Washington state agencies, health providers and community-based organizations. In-network partners act on referrals, participate in two-way care coordination and verify program information so capacity details remain current.
